Contact our Referrals Team via phone or email, and we will respond promptly to discuss the young person’s placement needs, preferences, and support requirements.
Step 3: Initial Placement Plan & Risk Assessment
Upon confirming suitability, we develop an initial placement plan and risk assessment, outlining how we will:
✔ Provide tailored care and safeguarding
✔ Work collaboratively with professionals involved
✔ Support the young person’s emotional and social development
Step 5: Placement Confirmation & Transition Support
Once agreed, we coordinate the admission process, ensuring the young person is welcomed into a safe, supportive, and nurturing environment.
Step 2: Assessment & Placement Matching
We carefully review the referral details and select the most suitable setting based on:
✔ Age and developmental needs
✔ Health and medical requirements
✔ Behavioral and emotional support needs
✔ Educational background and EHCP (if applicable)
Step 4: Pre-Placement Visit
We facilitate pre-placement visits for the young person, social workers, and family members to ensure a smooth transition and best possible fit.
Documents Required for Referral
To process a referral efficiently, please attach the latest supporting documents, including:
✔ CLA (Child Looked After) Reports
✔ Health Assessments & EHCP (if applicable)
✔ Chronology & Risk Assessments
✔ Any additional reports from professionals involved
